To register for the 2008-2009 season, you must fill out the following forms:
- 2008-2009 Parent/Swimmer Contract [29 KB PDF]
- 2008-2009 Emergency Information Form [37 KB PDF]
- 2009 U.S.A. Swimmer Registration [27 KB PDF]
- Only if you are transferring from another year-round or winter season team, will you need to complete a club transfer form:
- 2008-2009 Ohio Swimming Transfer [13 KB PDF]
Please complete one copy of each form per swimmer and mail them to:
