- To register for the 2007-2008 season, you must fill out the following forms:
- 2007-2008 Parent/Swimmer Contract [PDF]
- 2007-2008 Emergency Information Form [PDF]
- 2008 U.S.A. Swimmer Registration [PDF]
- Only if you are transferring from another year-round or winter season team, will you need to complete a club transfer form:
- 2008 Ohio Swimming Transfer [PDF]
Please complete one copy of each form per swimmer and mail them to:
